Four Infants Hospitalized from Formula

Abbott voluntarily recalled several of its baby formula products after four infants reportedly got sick and hospitalized with bacterial infections. The powder formulas were distributed across the country, and possibly exported to other countries, the Food and Drug Administration said.

The powder formulas impacted by the recall include Similac, Alimentum and EleCare. To identify if you have a package affected by the recall, check the number on the bottom of the container. If it starts with digits 22 through 37 and contains K8, SH or Z2, and has an expiration of April 1, 2022, or later, it should be thrown out. All of the recalled formula was produced at the company’s Sturgis, Michigan, facility, the company said.

Three of the complaints concerned Cronobacter sakazakii, a bacterium that can cause severe, life-threatening infections or inflammation of the membranes that protect the brain and spine. Cronobacter infection may also cause bowel damage and may spread through the blood to other parts of the body.

The F.D.A. had posted a recommendation on Thursday to parents warning them about the products after illnesses in Minnesota, Ohio and Texas resulted in the hospitalization of four infants. The bacterium may have contributed to a death in one case

Piedmont woman dead after semi-truck collision involving three vehicles.

One person is dead and another is critically injured in Logan County after three-car wreck involving a semi-truck.

45-year-old Gayle Lynn Lanoy of Piedmont was westbound on Oklahoma 33 in a 2021 Toyota 4Runner about 8:30 a.m. when she attempted a legal stop on the two-lane stretch of highway to make a left turn.

A westbound tractor-trailer rig driven by a 57-year-old Oklahoma man struck the 4Runner, the report states, and Lanoy’s vehicle then crossed into the path of a 2017 GMC pickup. Lanoy was ejected about 30 feet when the eastbound pickup struck her vehicle, troopers said.

Lanoy was pronounced dead at the University of Oklahoma Medical Center in Oklahoma City. She was initially taken to Mercy Medical Center in Guthrie but was later flown to the OKC hospital. She died from head, arm, leg and internal and external torso injuries, according to Oklahoma Highway Patrol officials.

The driver of the pickup, a 39-year-old Edmond man, was pinned about a half hour before being extricated by Guthrie firefighters. He was also transported by air to an Oklahoma City hospital; troopers report he was in critical condition.

The driver of the tractor-trailer rig was not injured.

The cause of the crash and further details remain under investigation, according to Oklahoma Highway Patrol’s preliminary collision report.

What Is a Window Blind Strangulation Lawsuit?

According to USA Today, approximately one child every month dies from strangulation by a window blind cord.

How Common Are Injuries and Deaths from Window Blind Cords?

A study by the American Academy of Pediatrics (AAP) using data from 1990 to 2015 indicates that there were approximately 16,827 injuries related to window blinds among children under six years of age. This is an injury rate of 2.7 per 100,000 children. Nearly 12 percent of those injuries were from entanglement and 98.9 percent of those entanglement injuries involved window blind cords. The vast majority of entanglements (98.7 percent) involved the child’s neck, and a significant number (67.1 percent) of entanglement incidents resulted in death.

The United States Consumer Product Safety Commission (CPSC) lists window coverings with cords as one of the top five hidden household hazards. According to the report, these cords can present a danger to children up to 8 years old. The CPSC has recalled over five million window coverings, including blinds and Roman shades. 

Are All Window Blinds Dangerous for Children?

The AAP report indicates that window blind cords are most dangerous to toddlers, but they present a risk to all children under six years old. It’s extremely easy for naturally curious toddlers and children to unintentionally tangle themselves in the cords. Additionally, strangulation injuries and deaths are silent and very quick; most incidents occurred when a child was left alone for less than 10 minutes.

According to the AAP, most incidents involved horizontal blinds. In these incidents, the operating cords were involved in about 71 percent of cases, and 17 percent were related to inner cords. 

However, other types of window coverings and cords have also caused injuries and deaths. In cases involving vertical blinds, most of the incidents involved the continuous loop cord. The inner cords on the back of Roman shades were responsible for 85.2 percent of incidents involving that type of window covering.

How Does Liability Work in Window Blind Injury or Death Cases?

The dangers of window blind cords have been known for decades, but most safety standards for these products are voluntary. As such, there are many manufacturers that have chosen not to make design changes (e.g., cordless blinds) to comply with these non-mandatory standards. 

Many window blinds and shades are still being made and sold with cords that present a strangulation risk to children. When incidents involving these dangerous products occur, it is possible to file a personal injury lawsuit to hold the manufacturer liable.

A liability case may be based on the fact that manufacturers have known about the dangers of their products for decades and have chosen not to implement design changes to improve safety and reduce risk.

What Type of Compensation Can You Sue For?

In a personal injury case, the plaintiff may seek compensation for specific expenses as well as emotional damages. For example, if a child was injured from an entanglement, the lawsuit may seek compensation for the medical bills related to diagnosis and treatment. In the event of a fatal incident, there may be compensation to cover funeral arrangements.

Additional compensation may be related to the child’s and/or parents’ pain and suffering and the child’s lost future wages (if there is a permanent disability). A judge may also assign punitive damages to encourage the manufacturer to make safer products.

Consumers should immediately stop using the recalled vehicles and contact a Polaris dealer to schedule a free repair, including the installation of a new fuel line. Polaris has notified consumers to stop riding the vehicles. Polaris will be contacting all registered owners directly to provide more details about the repair.

This recall involves certain VINs of Polaris Model Year 2022 Outlaw 110 EFI Youth ATVs. The recalled ATVs were sold in gray/lime and gray/pink color combinations. “POLARIS” is printed on both sides of the vehicle below the headlights and on both sides of the seat. “Outlaw 110” is printed on both sides of the chassis. The vehicle identification number (VIN) is located in the left side wheel well.

California Woman Dies from Injuries Related to Crash in Tulsa

A California woman has died due to injuries she suffered just over a month earlier in a crash in Tulsa, according to the Oklahoma Highway Patrol.

Leticia Lopez, 44, of Fullerton, California, was hospitalized at St John Medical Center in Tulsa, in critical condition November 25, 2021 after a crash involving another vehicle along Interstate 244 near Harvard Avenue about 4 a.m.  She died Wednesday, December 29, 2021.

Fatal Collision in Perkins, OK

A nine-year-old boy has been pronounced dead at Stillwater Medical following a collision on State Highway 33 and Fairgrounds Road, near the Liberty Church of Perkins and Frank’s Diesel Service. The child, whose name has not been released as of yet, sustained head and trunk internal injuries reports Oklahoma Highway Patrol. 

According to the OHP report, Sarah Davidson, 31, of Cushing, was driving a 2015 Honda Pilot carrying four children from Perkins. 

Stillwater News Press reports, Davidson was transported by Cushing Fire to OU Medical with head and trunk internal injuries. One of her passengers, an 11-year-old boy, was transported by Cushing Fire to OU Medical with head injuries.

This recall involves Backyard Play Systems outdoor playsets with a wooden roof. The wooden roof is included with home play equipment marketed under the brand names Yardline Play Systems (models Turbo Racer, Cloud Racer) and Gorilla Playsets (models Captain’s Fort, Fort Highlander). The playsets include a solid wood roof with a lookout window, slides, swings, a rock-climbing wall and other play features.

The wooden parts used to reinforce and create a decorative gable design on the wooden roof pose an entrapment hazard to children. Children can become trapped in the gap between the lower part of the roof structure and the bottom of the gable decoration.

Fatal Car Crash – Saturday, November 6th

A man was fatally crushed between two cars just after midnight on Saturday, November 6th in Oklahoma City.

The crash happened on SW 3rd Street and MacArthur near Walmart at about 12:30 a.m.

The man crashed into the rear of another vehicle, he was then standing in front of his vehicle and behind the struck car. A third driver then crashed into the back of the man’s vehicle, causing him to be crushed between his vehicle and the rear-ended car.

The man was pronounced dead after the incident. He was not immediately identified. At this time we are unclear if the other drivers were hurt.

Walmart Recalls Better Homes and Gardens Essential Oil Infused Aromatherapy Room Spray with Gemstones Due to Rare and Dangerous Bacteria; Two Deaths Investigated

This recall involves Better Homes and Gardens Essential Oil Infused Aromatherapy Room Spray with Gemstones. Tests conducted by the Centers for Disease Control and Prevention (CDC) determined that a bottle of the room spray contained the rare and dangerous bacteria Burkholderia pseudomallei which causes melioidosis, a condition that is difficult to diagnose and can be fatal. CDC has been investigating a cluster of four confirmed cases of melioidosis including two deaths in the U.S. Though the source of these four infections has not been confirmed by CDC, the bottle with the same type of bacteria was found in the home of one of the melioidosis decedents.

Phoenix 200 All-Terrain Vehicles ATVs Recalled by Polaris Due to Crash Hazard– About 13,000 sold

In September 2021, Polaris recalled the Phoenix 200 All-Terrain ATVs, all model years 2011 through 2021. They were sold in red, blue or gray. “Polaris” is decaled on the front body and the sides of the ATV seats and “Phoenix 200” is decaled on the side panels. The Vehicle Identification Number (VIN) is located on the vehicle’s front left frame.

About 13,000 of these ATVs were sold at Polaris dealers nationwide from September 2010 through June 2021 for between $3,500 and $4,300. The firm has received 18 reports of throttle stop breaking or sticking, including one report of a crash which resulted in a minor injury.

Tulsa 911 dispatcher killed in hit and run

A hit and run collision at around 9:15pm on Tuesday, October 5 at Admiral Boulevard and Utica Avenue in Tulsa has left a longtime 911 dispatcher dead. The intersection at Admiral and Utica hosts Admiral Park and Robertson Tire.

Mary Byers served Tulsa for 26 years as a 911 call taker, dispatcher, and supervisor.

Tulsa police say the man alleged to have hit her, 27-year-old Cody Carrell, turned himself in later, on Tuesday. In an arrest report it was stated Carrell ran a red light and hit Byers car. Speed is believed to be a factor in the accident.

Teen hits motorcyclist in Tulsa, OK

A 16-year-old boy driving a white sedan struck and badly injured a motorcyclist on 61st St near Hwy 169 Monday due to road rage.

The teen was trying to merge onto Hwy 169 from 81st St, while a woman driving north on Highway 169 tried to exit at 71st. The 16-year-old driving a white car was getting on the highway at 81st and forced her to swerve to avoid hitting him. He then threw a water bottle at her car.

Washington, D.C. – The U.S. Consumer Product Safety Commission (CPSC) and The Boppy Company, of Golden, Colorado, are announcing the recall of the Boppy Original Newborn Loungers, Boppy Preferred Newborn Loungers and Pottery Barn Kids Boppy Newborn Loungers. There have been eight reports of infant deaths associated with the Boppy Company Newborn Lounger and this hazard. The infants reportedly suffocated after being placed on their back, side or stomach on the lounger and were found on their side or on their stomach. The infant deaths occurred between December 2015 and June 2020.

Harbor Freight Tools Recalls to Repair Propane Portable Heaters Due to Fire Hazard – About 19,000 sold

On September 16, 2021, Harbor Freight has recalls to repair their propane heaters due to fire hazards. The fitting at the back of the heater can fail to be sufficiently tight, causing the heater to leak propane gas, posing a fire hazard.

About 19,000 of these heaters were sold at Harbor Freight Tools stores nationwide and online at www.harborfreight.com from November 2020 until March 5, 2021 for about $100. The firm has received 13 reports of propane gas leaking, including one report of a fire. No injuries have been reported.
