Spalding® Momentous EZ Portable Basketball Goals Recalled by Russell Brands Due to Impact Injury Hazard- over 13,000 units sold
Consumers should immediately stop using the portable basketball goals and contact Spalding to arrange for a technician to remove the original arm and install a new arm on the goal system. Spalding is contacting all known purchasers.
This recall involves Spalding Momentous EZ Assembly Portable Basketball Goals. The goals have a 50 inch or 54-inch acrylic clear backboard with a visible “H-frame” providing support behind the backboard, or a 54-inch acrylic clear backboard with aluminum trim, but no H-frame. All backboard styles are attached to a black metal pole with a black metal arm. “Spalding” is printed on the front of the black base and “6E” is molded on the back. The numbers 1010, 1011 or 1012 are printed to the right of the CAUTION statement on the front of the backboard.
The firm has received 26 reports of weld failures including three backboards separating from the pole. One injury involving the backboard hitting a consumer on the shoulder/upper torso and one injury hitting a consumer on the head has been reported.

Mr Maria Recalls Miffy First Lights Due to Burn Hazard
This recall involves Miffy First Lights with a blue color label on the bottom. The lights are about 12 inches tall and are made from silicone and have a white exterior in the shape of a rabbit. The LED light has a step dimmer and USB charger. The model number Mr LED 11 or Mr. LED 11A is printed on the blue-colored label on the bottom of the light. The recalled lights were produced in 2017 and 2018. The production year is embossed in a circle on the bottom right foot of the light; a “7” on the right side of the arrow represents 2017 and an “8” on the right side of the arrow represents 2018. Models with only a blue-colored label on the bottom of the unit are included in this recall.
Consumers should immediately stop using the recalled Miffy First Lights and visit https://www.mrmaria.com/bluemodule for instructions on how to determine which LED module is being recalled. Consumers who purchased the Miffy First LED Light at a local store, should bring the light to the store to have the module replaced free of charge. Consumers who purchased the light online (or if the retailer is no longer in business) should take one photo of the blue label on the bottom of the light and one photo of the date embossed underneath the right foot of the light and email both photos together with their shipping address and phone number to bl********@mr*****.com to receive an upgraded LED module free of charge.

Consumers should immediately stop using the recalled ceramic mugs and contact Accompany USA to receive a full refund.
This recall involves Accompany USA Ceramic Mugs with Cork Bottoms sold as and/or given out for free as promotional products with company or other logos. The ceramic mugs are white with cork bottoms. They measure about four inches high and four inches wide.
Accompany USA has received one report of the mug breaking after hot liquid was poured into it. No injuries have been reported.

Studies have shown that taking Tylenol or other private label APAP during pregnancy increases the risk of having a child with Autism Spectrum Disorder and/or ADHD.

In 2018, researchers did a meta-analysis of seven studies involving 132,738 pairs of mothers and children. The analysis revealed a 20% higher risk of autism for children who had prolonged exposure to acetaminophen in the womb.

In 2019, Johns Hopkins Bloomberg School of Public Health found that children whose cord blood samples contained the highest levels of acetaminophen — the generic name for the drug Tylenol — were roughly three times more likely to be diagnosed with autism spectrum disorder later in childhood, compared to children with the lowest levels of acetaminophen in their cord blood.

Marine Corps Base Camp Lejeune – North Carolina Water Contamination Claims for Compensation

TJX Recalls Egg Chairs Due to Fall Hazard; Sold at Marshalls, T.J. Maxx, HomeGoods and Homesense Stores- over 30,600 units sold

Consumers should immediately stop using the recalled egg chairs and return them to any Marshalls, T.J. Maxx, HomeGoods, or Homesense stores for their choice of either a full refund or a refund in the form of a store gift card. Consumers can also contact TJX for instructions on how to disassemble and dispose of the chair to receive either a full refund or refund in the form of a store gift card.

This recall involves nest swing egg chairs sold under the Tommy Bahama and Martha Stewart brands. The chairs have a metal circle base that attaches to a metal pole, from which the oval-shaped wicker chairs hang by hook and chain. The chairs were sold with a round cushion for seating and a hangtag with the brand name “Martha Stewart” or “Tommy Bahama.”

Eight of the nine styles were also sold with an additional decorative pillow. One of the following style numbers is printed on a second hangtag attached to the product: PMK-6501, PMK-6503, PMK-6503-N, PMK-6505, PMK-6506, PMK-6507, PMK-6508, PMK-6509, or PMK-6510.

The firm has received 27 reports of the chairs collapsing or tipping over, including 19 reports of injuries, including cuts, scrapes, soreness and one report of broken ribs and a collapsed lung.

All purchasers and installers should immediately contact Schneider Electric to arrange to have the recalled load centers inspected by trained electricians to determine if replacement or repair is required. This inspection and any resulting replacement or repair is free of charge.

Consumers can find the catalog number and date code on indoor load centers inside the door of the panel. Sienhua Group Recalls WarmWave and Hunter Ceramic Tower Heaters Due to Fire and Burn Hazards – over 580,000 units sold

Consumers should immediately stop using the recalled heaters, unplug the unit, and follow the instructions online at (www.homeproductservice.com/recall) on how to properly sever the electrical cord to make the unit unusable, and contact the distributor, Sienhua Group (North America), to receive a prorated refund.

The firm has received eight customer reports of fire at the plug or cord and dozens of customer reports of the plug or cord overheating. There have been no reports of injury or death and one report of minor property damage.

Sold at Home Depot stores nationwide and online at Amazon.com, eBay, and other websites from September 2013 through February 2022 for about $30 to $40 depending on when purchased.

Trucking Accidents by the Numbers 2022

Trucking Accident Statistics in Oklahoma

According to the Oklahoma Highway Safety Office (OHSO), there were a total of 4,784 crashes in the state in 2020 (the most recent year for which data is available) that involved large trucks. Those crashes resulted in 75 fatalities and 1,563 injuries.

National Trucking Accident Statistics

Data from the National Safety Council indicates that in 2020, large trucks accounted for 9 percent of all vehicles involved in fatal crashes despite the fact that they were only 4 percent of all registered vehicles. In the same year, 107,000 large trucks were involved in injury crashes throughout the U.S.

Nationally, most fatal crashes involving large trucks in 2020 occurred in rural areas, on non-interstate roads, and during the day. The highest number of crashes happened in October, and April had the fewest large truck crashes.

Common Causes of Trucking Accidents in Oklahoma

According to the OHSO, 29 percent of the truck crashes that happened in Oklahoma in 2020 occurred on interstate highways. Additionally, 24 percent of large truck crashes happened on city streets, and 13.1 percent occurred on county roads. The majority of large truck crashes (approximately 70 percent) involved two vehicles.

The OHSO also provides details about the causes of trucking accidents in the state. For truck drivers, the most common factors that contribute to an accident are the following:

  • Improper act/movement (e.g. improper backing, failure to signal, improperly secured load, sleepiness, etc.)
  • Speeding
  • Unsafe lane changes
  • Failure to pay attention
  • Improper turning

For drivers of other vehicles that were involved in large truck crashes, these were the most common contributing factors:

  • Failure to yield
  • Unsafe lane changes
  • Improper act/movement (e.g. ignoring warning signals, sleepiness, failure to signal, etc.)
  • Inattention
  • Speeding

When it comes to injuries and fatalities in large truck accidents, it often doesn’t matter who was at fault for the accident. According to the Insurance Institute for Highway Safety, passenger vehicle occupants account for most deaths in large truck crashes, simply because trucks have a higher clearance and weigh significantly more.

2 people injured and 1 dead after motorcycle wreck in Catoosa, OK

Motorcycle Wreck Carr

Reports of a deadly crash involving two motorcycles and a car on Sunday night around 10 p.m.
According to Oklahoma Highway Patrol (OHP) troopers, the two motorcycles crashed along I-44, near 129th East Avenue, just before 10 p.m.

Troopers say a car then hit one of the motorcycles. 30-year-old Shelbi Knapp from Catoosa died in the crash.

The Oklahoma Highway Patrol says Knapp’s motorcycle, another motorcycle with a 32-year-old man and a juvenile passenger, and a 2005 Chevrolet Malibu were involved in the crash.

Polaris Recalls RZR Recreational Off-Road Vehicles Due to Fire Hazard – over 1,000 units sold

Consumers should immediately stop using the recalled vehicles and contact a Polaris dealer to schedule a free inspection and repair. The dealer will re-route the battery cable, and if needed, replace it free of charge. Polaris has notified consumers to stop riding the vehicles and is contacting registered owners directly to provide more details about the repair.

This recall involves certain VINs of Model Year 2022 RZR Pro R 4 Premium and Ultimate vehicles. The recalled vehicles were sold in the following colors: black, blue and lime.

The recalled vehicles were sold in a four-seat configuration. POLARIS is stamped on the front grille and POLARIS and RZR are printed on the sides of the vehicles. The model name is also printed on the side of the chassis. The VIN is located on the vehicle’s frame in the left rear wheel well.

The firm has received one report that a vehicle lost power after start up. No injuries or fires have been reported.

Sold at Polaris dealers nationwide from May 2021 through April 2022 for between $39,000 and $45,000.

Dollar Tree Recalls More than One Million Hot Glue Guns Due to Fire and Burn Hazards– over 1 million units sold

Consumers should immediately unplug and stop using the Crafter’s Square Glue Gun and return it to any Dollar Tree or Family Dollar store for a full refund. Online purchasers will be contacted directly with further instructions.

This recall involves Crafter’s Square Glue Guns. They dispense hot glue when plugged into an outlet and the trigger is depressed. The plastic glue gun is black with an orange trigger and tip. The glue guns have a silver UL listed label located above the handle with “GLUE GUN” and “XY-15302” printed on the label.

Dollar Tree has received seven reports of electrical malfunctions when using the glue guns, including four reports of fire and one report of skin irritation.

Sold at Dollar Tree stores nationwide from August 2020 through February 2022 and Family Dollar stores nationwide from January 2022 through February 2022 and online at www.dollartree.com from August 2021 through February 2022 for about $1.

Rae Wellness Recalls Prenatal and Immunity Dietary Supplements Due to Failure to Meet Child Resistant Packaging Requirement; Risk of Poisoning

Consumers should immediately store the recalled products in a safe location out of reach of children and contact Rae Wellness for information on how to dispose of the product and receive a full refund. Consumers can also contact the place of purchase for a full refund.

This recall involves bottles of Rae Wellness Prenatal and Immunity Capsules. Both products were sold in 60-capsule bottles and contain iron and have “Rae” in yellow lettering and “Prenatal” or “Immunity” printed on the label. The bottles have a white continuous thread closure.

Sold at Target stores nationwide and online at Target.com, ThriveMarket.com, Amazon.com, FabFitFun.com and StandardDose.com from September 2019 through February 2022 for the Rae Prenatal Capsules and from October 2020 through February 2022 for the Rae Immunity Capsules, for about $15.

Telebrands Recalls Hempvana Pain Relief Products with Lidocaine Due to Failure to Meet Child Resistant Packaging Requirement; Risk of Poisoning

Contact Telebrands for information on how to safely dispose of the recalled products and how to receive a full refund or a free child resistant closure for the jar products. In addition, Telebrands is providing a $15 digital coupon code good toward the purchase of any Hempväna products at www.Hempvana.com. The products contain the substance lidocaine which must be in child resistant packaging as required by the Poison Prevention Packaging Act (PPPA). The packaging of the products is not child resistant, posing a risk of poisoning if the contents are swallowed or placed on the skin of young children.

This recall involves Hempväna pain relief products containing lidocaine, including Hempväna Ultra Strength Pain Relief Cream with Lidocaine, Hempväna Ultra Strength Pain Relief Cream Night with Lidocaine, Hempväna Ultra Strength Pain Relief Cream Turmeric with Lidocaine, and Hempväna Pain Relieving Hand & Body Lotion with Lidocaine. “Hempväna” and “Pain Relief” or “Pain Relieving” is printed on the packaging in large black letters.

PlayMonster Recalls Kid O Hudson Glow Rattles Due to Choking Hazard – about 8,920 units sold

Consumers should immediately take the recalled rattles away from children, stop using them, and contact PlayMonster for instructions on receiving a $25 refund. PlayMonster will provide consumers with a prepaid shipping label to return the recalled product.

This recall involves the Kid O Hudson Glow Rattle, a motion-activated rattle shaped like a puppy that makes a soft rattling sound when shaken. The plastic puppy is white with spots that can glow in red or green. The puppy’s legs are textured soft plastic in red (front) and green (back) for teething children. The recalled rattles measure about 4 inches long by 2 inches wide by 3.8 inches high. The battery cover on the puppy’s stomach states “Kid O.”

The firm has received three reports of the rattle legs breaking off. No injuries have been reported.

The PlayMonster was sold at specialty stores nationwide and online at amazon.com, walmart.com, playmonster.com, and zulily.com from February 2018 through February 2022 for about $25.

SOUNDBOKS Recalls Bluetooth Speakers with Lithium-Ion Batteries Due to Fire Hazard (Recall Alert)– over 6,700 units sold

Consumers should immediately stop using the recalled speakers, remove the battery and contact the firm to receive a new battery including shipping. SOUNDBOKS will provide free replacement lithium-ion batteries and a $100 credit toward purchases at SOUNDBOKS web shop, as well as instructions for properly disposing of lithium-ion batteries according to federal and state regulations. SOUNDBOKS has notified all known owners directly to stop using the recalled speakers.

This recall involves SOUNDBOKS (Gen. 3) Bluetooth performance speakers with BATTERYBOKS 3 lithium-ion batteries. The speaker measures about 26.5 inches high by 17 inches wide by 13 inches deep. The grill cover of the speaker was sold in black, white, orange, red or blue. The battery is removable from the speaker and connects through a slot on the side of the speaker, opposite the user interface. Each speaker has a unique serial number printed in the slot-in compartment where the battery goes. Consumers should reference the speaker serial number when contacting SOUNDBOKS.

Sold Online at SOUNDBOKS.com, Amazon.com, and DJ Tech Tools.com from November 2021 through February 2022 for about $1,000.

Consumers can also go to https://tonies.happyreturns.com/ and choose “Blocks Return” as the reason for return. tonies will then provide the customer with a return label. After the product has been received, tonies will provide the initial form of payment with a full refund and an additional $14.99 tonies.com coupon code. tonies is also contacting all purchasers directly. The recalled products are being remotely deactivated so that they no longer function.

Tonies has received one report of the magnet detaching from the product. No injuries have been reported.

This recall involves tonies wooden blocks, with magnets inside. Only tonies Blocks Marine Life, UPC 840147407689, tonies Blocks Invention and Innovation, UPC 840147407733 and tonies Blocks Gentle Giant Dinos UPC 840147407580 are being recalled.

Each block is about 5 cm wide and 1.5 cm thick. The blocks were sold in blue, tan and green with pictures on one side of various marine life, dinosaurs, and invention and innovation, and white text on the other side, describing the picture. Blocks were sold in sets of four. Each set is packaged in a white and red cardboard box. The UPC is on the back of the box beneath the barcode.

They were sold exclusively online at tonies.com from November 2021 through December 2021 for between $15 and $20.

Commencal Recalls Ramones 14-Inch Kids Bicycles Due to Crash Hazard

This recall involves the Commencal Ramones 14-inch children’s bicycles. The single speed bicycles have handbrakes but do not have footbrakes. The bicycles were sold in multiple colors with the model year printed on the front fork leg stickers.

No incidents have been reported yet.

Sold online at www.commencalusa.com from December 2014 through March 2021 for between $230 and $320.

EGL Motor Recalls EGL and ACE-branded Youth All-Terrain Vehicles (ATVs) Due to Violations of Federal ATV Safety Standard; Risk of Serious Injury or Death

Consumers should immediately stop using the recalled ATVs and contact EGL Motor to make an appointment for a free repair from an authorized repair shop. Reflectors, headlights and a chest protector will be installed on the ATV. EGL will also repair the parking brake, speed capability and spark arrestor.

The ATVs fail to comply with the requirements of the federal mandatory ATV safety standard. The B125 ATVs exceed the maximum speed limitations for vehicles intended for children aged 10 and older. For the Madix 110, the parking brakes do not prevent the movement of the vehicle. In addition, the handlebars on both ATVs pose a laceration hazard should the rider’s body or head impact the handlebars at a high rate of speed such as in a crash. ATVs that fail to meet the mandatory safety requirements pose a risk of serious injury or death.

The recall involves EGL and ACE-branded Youth ATVs, models MADIX 110 and B125. The vehicles were sold in various colors including orange, red, blue, pink, and green and marketed for children ages 10 and older. The name of the vehicle is printed on a label located on both sides of the vehicle. The model number is stamped into the metal plate located on the left side, under the rear body. The phrase “This ATV is subject to EGL MOTOR INC’s action plan approved by U.S. Consumer Product Safety Commission” is printed on a label located under the model number plate.

No incidents have been reported yet.

American Honda Recalls Recreational Off-Highway Vehicles Due to Crash and Injury Hazards: – over 32,000 units sold

Consumers should immediately stop using the recalled recreational off-highway vehicles and contact an authorized Honda Powersports dealer to schedule an appointment for a free inspection and repair. Honda is contacting all known purchasers directly.

The vehicle’s intake funnel band screw can loosen and enter the engine through the intake funnel and cause sudden engine failure. If this occurs, it could result in a loss of control, increasing the risk of a crash or injury. Honda has received 16 reports of loose intake funnel band screws entering the engine, resulting in engine damage/failure. Honda has received one report of a loss of control, resulting in a crash with minor injuries to the occupants.

Sold at authorized Honda Powersports dealers nationwide from March 2019 through January 2022 for between $20,000 and $27,000.

This recall involves model year 2019 – 2021 Honda Talon 1000 S2 and S4 recreational off-highway vehicles (ROVs). The vehicles were sold in various colors including red, blue, green, gray and yellow. “HONDA” is printed on the front, sides and the rear of each vehicle. “Talon” is printed on a label located on both sides of the vehicle.

HD Premier Recalls DigitDots Magnetic Balls Due to Ingestion Hazard – about 119,620 units sold

Consumers should stop using the recalled magnets immediately, take them away from children and contact HD Premier to receive a pre-paid label to return the recalled products.  Upon receipt of the products, consumers will receive a refund.

This recall involves 3mm and 5mm DigitDots Magnetic Balls which are small, spherical, loose and separable neodymium rare-earth magnets with a strong magnetic flux.  The magnets are sold in two sizes: 3mm and 5mm.  The 3mm diameter magnets are sold as aggregated loose magnets in sets of 512 multi-colored magnetic balls.  The 5mm diameter magnets are sold as aggregated loose magnets in sets of 222 silver magnetic balls or 224 multi-colored magnetic balls. The recalled DigitDots were sold encased in clear, disposable packaging bearing the “DigitDots” name and logo and included a plastic carrying case, also bearing the “DigitDots” name and logo.

HD Premier is aware of four children who have ingested DigitDots and required surgery to remove the magnets.  In addition, CPSC is aware of other reports of children and teenagers ingesting other companies’ high-powered magnets and requiring surgery, including two deaths.

The magnetic balls were sold at www.ilovedigitdots.com, www.amazon.com and other websites from March 2019 through January 2022 for between $20 and $30.

Dangers of Spray Foam Exposure

Polyurethane spray foam is a two-part compound that is mixed on site. Two types (open cell and closed cell) are available from many different manufacturers, under more than a dozen product names:

Manufacturers: Icynene Lapolla, BASF, NCFI Polyurethanes, Johns Manville, Dow, Demilec, Huntsman

Open cell spray foam: Enertite, Sealection 500, Agribalance, Demilecapx, Classic, Classic Ultra, Ultra Plus, Foam-Lok, JM Corbond, Sealtite

Closed cell spray foam: Walltite, Heatlock, Pro Seal, Foam-Lok 2000, JM Corbond IV, Insul Star, Insul Bloc

Spray foam insulation has resulted in acute and long-term exposure to toxic and harmful chemical gases. Spray foam insulation has been found to contain formaldehyde, a cancer-causing substance. A powerful chemical reaction takes place when spray foam insulation is mixed onsite that is not free of risk. Installers are required to wear full protective gear while working and ensure ventilation throughout the installation process and for the 24 successive hours.

The Safety Data Sheet also includes warning information indicating a risk for breathing / lung problems, liver, kidney, and nervous system damage, and birth defects from spray foam insulation. In cases where the chemicals were mixed incorrectly, the product served to trap its own hazardous fumes into the home and continued to off-gas over the course of years, creating a gas chamber. Occupants of such homes have developed severe and debilitating health problems, and their homes have been rendered uninhabitable.
